REGISTRATION

If you are interested in registering as an NHS patient or as a private patient, and live in Bradford or Leeds, then you can register your interest either online or by downloading and filling in the form below. A member of our staff will then contact you to provide further details. You can register as an NHS patient  if you live within our practice area boundary ( 1.5 miles from the practice); if you live outside our practice area boundary then acceptance of your registration is at the discretion of the practice.

You can also  download the relevant forms below and  bring it with you with a combination of list given below and we will then register you at the practice.  The practice will then book you for a new patient health check with one of our healthcare professionals

A combination of the following can be accepted as identification (it is preferable that one item of photo ID is seen, along with one document containing the patient’s address):

 

  • marriage certificate
  • birth certificate
  • medical card
  • driving licence
  • passport
  • local authority rent card
  • paid utility bills
  • bank/building society cards/statements
  • National Insurance number card
  • payslip
  • letter from Benefits Agency/benefit book/signing on card
  • papers from the home office
  • P45.

The following documents are easily obtained and are not accepted as proof of identity if presented in isolation:

  • library card
  • video rental card
  • health club card
  • private rent book.
