Report A Problem/Positive (RAP)

This feedback form is for Patients, Carers, any stakeholders and providers to share ideas, areas of good practice and areas for improvement within services that are provided by us. This information will be used to drive the quality of services and improve outcomes for patients.

We want to make sure that we are capturing all feedback that patients are giving us for our services and also staff approach towards our service users.

These areas of good practice/areas of development may arise from frontline staff, patient experience, ongoing care, and could be around access, correspondence, waiting times or any other areas of good practice/areas of development.

The feedback reported will be collected on a monthly basis into themes and will be shared with members of the management team to seek feedback.  We will discuss the reported themes in our monthly Quality Meetings.

This Form should NOT be used to report Serious Incidents complaints or issues requiring urgent resolution and patient identifiable information should not be entered.

The RAP process is about correlating your reported positives and problems and also ensures that you are treated well by the staff and clinicians.
